Initial Comments:
Based on the findings of an off-site follow-up survey conducted on July 12, 2021, Avalon At Home, LLC, corrected the deficiencies found in the re-licensure survey conducted May 11. 2021 and is in compliance with the requirements of 28 Pa. Code, Part IV, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries and Subpart A, Chapter 51.
